Entertaining a favorable opinion of the mission of The Knights of Saint Andrew, and being in full accord with its aims and objectives, I hereby accept the invitation to become a member of the Winston-Salem Chapter, and pledge myself to comply with the Constitution, By-Laws, and Regulations of the Knights of Saint Andrew. I enclose the required initiation fee of $25.00 and the first year's dues of $10.00 for a total of $35.00.
